Introduction
New York State Law requires any person who performs a Marriage Ceremony within the City of New York to register with the Office of the City Clerk prior to performing the Marriage Ceremony. Upon submitting this Marriage Officiant Registration Application, you will receive a Confirmation Number. To complete your application, you must
print
the application and mail it along with your Confirmation Number, your payment, proper identification, and any additional required documentation to the
Manhattan Office
of the City Clerk, 141 Worth Street, New York, New York 10013.
Before you mail in your application materials, you must have your signature notarized.
