Introduction
New York State Law permits amendments to be made to a Marriage Record if you claim that the mistake was made through inadvertence in any of the statements on your application. Upon submitting this Marriage Record Amendment Application, you will receive a Confirmation Number. To complete your application, you must
print
and sign your application and mail along with your Confirmation Number, your payment, proper identification, and any additional required documentation to the
Manhattan Office
of the City Clerk, 141 Worth Street, New York, New York 10013.
Before you mail in your application materials, you must have your signature notarized.
