Introduction
So long as both you and your prospective partner are 18 years old or older, you can fill out this form to submit your Domestic Partnership application to the Office of the City Clerk online. Upon submitting this Domestic Partnership application, you will receive a Confirmation Number. To complete your application, you and your prospective partner must request an appointment to appear together in person at the Office of the City Clerk. No other party may apply on behalf of either partner. You must bring your Confirmation Number, payment, proper identification, and any additional required documentation to the Office of the City Clerk.
